Psychiatric Rehabilitation Program for Adults (PRP)

Our PRP for Adults provides various services to individuals with mental health disorders to keep them functioning in the Community, thereby reducing homelessness and enhancing stability. The primary objective of our adult services is to assist clients in the following target domains:

  • Housing: During the initial session in the client’s home or Community, we will engage with them and their families to build housing planning and exploration. We will help the client with their home requirements. If a client becomes homeless, we will meet with them in the Community to help them connect with services and complete the necessary applications. Throughout the program, we will assist the client in achieving the long-term goals of financial stability and independent residence in the participant’s preferred Community.
  • Employment: We will meet with the customer to determine their professional interests and objectives. We will support the client throughout employment training by assisting with mock interviews and resume development, evaluating progress toward vocational goals, and resolving day-to-day client difficulties. In addition, we will support the client with job applications and follow-up while they seek independent work.
  • Transportation: We will assist the customer in identifying transportation requirements and obstacles to car independence.
  • Wellness Management: We will assist the customer in determining their substance misuse, mental health, and physical health requirements. We will assist the client in connecting with treatment providers to address these needs and collaborate with treatment providers throughout the program to ensure these needs are met.
  • Legal Support: We assist the client in developing a recidivism prevention strategy. When necessary, our employees will work with independent legal counsel.
Behavioral Health Services - Female intake
  • Family and Community Supports: We will assist the customers in identifying their leisure interests, family concerns, and a plan for community stabilization. In addition, we will engage the client in culturally appropriate activities and teach family-supportive social skills.
  • Education: We will assist the client in determining educational interests and objectives. CSI will lead the client throughout academic instruction and assist the client in autonomously pursuing more scholarly resources.
  • Entitlements, financial assistance, and budgeting: We will assist the client in identifying financial requirements. We will assist the customer in gaining access to benefits and financial aid and monitor applications and re-certification dates. In addition, our field team will be taught to help the customer formulate a budget to achieve financial stability.
  • Therapy compliance: We will work with the client to guarantee active participation in therapy and continued contact with psychiatrist services. The CSI therapy compliance specialist will support the client in maintaining mental health counseling sessions.
  • Illness Management: Applied Care Services provides a comprehensive approach to client care; we will assist the client in keeping all medical appointments and addressing all medical issues.
